Neurologist explains sports-related concussions after Wemby fall

San Antonio Spurs forward Victor Wembanyama (1) hits his head on the court Tuesday during the first half of Game 2 of a first-round NBA playoff series between the San Antonio Spurs and the Portland Trail Blazers at Frost Bank Center in San Antonio. He sat out the rest of the game after entering the NBA’s concussion protocol.
A San Antonio neurologist is optimistic that Victor Wembanyama could recover in as soon as one to three days after suffering a concussion during Tuesday night’s game.
Wembanyama is going through the league’s multistep process for returning to the court after a hard fall in the second period that forced him from Game 2 of the team’s first-round playoff series with Portland. The Spurs star landed hard on his chin after being fouled by Jrue Holiday while driving to the basket in the first half.
Under the NBA’s concussion protocol, the soonest Wembanyama can be reevaluated for returning to play is 48 hours after his diagnosis. He then has to pass a series of benchmark medical tests before being cleared to return. According to NBA injury tracking data, the average length of time for a player to spend in concussion protocol is a week to 10 days.
We spoke with Dr. Ali Seifi, medical director of the Neuro-ICU at University Health and division chief of neurocritical care at UT Health San Antonio, to get more insight on sports-related concussions:
QUESTION: How severe would you categorize Wembanyama’s concussion?
ANSWER: The brain is like Jell-O inside the skull. Whenever you have a hit to the skull bone, that Jell-O starts to shake, and that shaking is a concussion. I cannot comment on (Wembanyama’s) situation directly because I am not his doctor, but I saw he hit his face and his right temple on the floor. After that, I saw he was able to say something, which is a good sign because he didn’t pass out, but of course, I did not see what happened when he got off the court. Most concussion patients recover within one to three days, and it seems likely the same could happen in this scenario.
Q: What does the NBA concussion protocol’s required return-to-participation process entail?
A: Usually, if the concussion happened during a contact sport, there is a mandatory requirement to be off the court for the first 48 hours. In the first 24 hours, they will be monitored and have tests done. The first day, they have to do some casual walks and daily activities. Then the second day will be some stationary activities. Gradually, after six to seven days, if they have no symptoms, like headache, nausea, seizures, weakness and changes in the eyes, then two physicians, one from the NBA and one from outside, should approve them to return to the games. This player will be out for at least 48 hours.

Victor Wembanyama sits on the court Tuesday after taking a hard fall during the first half of Game 2 of a first-round NBA playoff series against the Portland Trail Blazers in San Antonio. He was diagnosed with a concussion.
Q: Why is this an important protocol?
A: For the general public, this is important because if someone, even kids playing at school, gets another concussion within the first 48 hours of the first, this could lead to a severe impact on the brain. There’s a potential that the nerves and neurons of the cortex and the brain can be damaged. To avoid that, it’s safe to wait at least 48 hours.
Q: What long-term impacts can a concussion have?
**A:**If somebody has multiple concussions in a row with space in between, usually it does not impact the brain permanently. However, if someone is in a scenario where there are recurrent concussions to the brain monthly, that can have a long-term impact on the brain, as far as the memory and mood of the patient. People can become more anxious, depressed or foggy. So that’s why, for the sake of each of these players, the recommendation is to wait at least two days to avoid these long-term impacts.
Q: How often do you believe players go into the NBA concussion protocol compared with the NFL?
A: The NFL has many more concussions than the NBA. Based on the statistical data, there are hundreds more concussions per season in the NFL, like approximately 500-plus. For the NBA, it’s less studied because numbers are really low and happen one or two a season. Football is more of a contact sport and more aggressive than basketball. For (Wembanyama), he didn’t hit someone else’s head. His head hit the floor, which was unfortunate.
Basketball courts are like hardwood floors, which can have a high impact, especially if you’re running and hit your head. Also, if you fall from a higher height, you’re going to have a harder impact.
Q: Is there anything you want to tell Spurs fans about the process or sports-related concussions in general?
A: We wish Wembanyama a speedy recovery and will be happy to help if needed. All the physicians, clinicians and health care workers at University Health and the University of Texas San Antonio love the Spurs and are very proud, especially of (Wembanyama). For the younger kids watching who want to be professionals, it’s good that they consider being cautious. For parents, if your child has a concussion, follow the protocol and don’t panic. Concussions don’t have a special treatment or medication. The best thing you can do is rest. Sleep is not bad for concussion patients, as many people think. If the brain rests, it has a better recovery. If you’re worried, you can gently wake them up every one to two hours to make sure they’re OK.
Seek medical attention if they’re nauseous, throwing up, have seizures or abnormal shaking, worsening headaches or their pupils enlarge. Otherwise, they can recover at home.
